Thank you for your response and you have posed a relevant question. The most widely used Broset Violence Checklist (BVC) doesn’t discern patients by diagnosis, but rather presentation of symptoms. Behaviors that are considered indicators for violent outbursts in the BVC include: confusion, irritability, boisterousness, verbal attacks, physical attacks, and attacks on objects (Rechenmacher et al., 2014). These symptoms have some amount of commonality between psychosis and personality disorders. The more of those behaviors a patient demonstrates, the higher risk level for violent outburst. The benefit of this type of nursing assessment is that once identified at risk for violence, the nurse and associate staff are driven to implement additional interventions to mitigate that risk. Methods for decreasing agitation may include limit setting, de-escalation, one to one interaction, removing potential weapons from area, 15-minute observation, medications, or in more extreme instances, restraint and seclusion (Sarver et al., 2019). That all being said, and returning back to your question, I would think that the patient with personality disorder would be more responsive to increased nursing interventions resulting in decreased episodes of aggressive or violent behaviors. Confusion versus intentionality is a discerning factor between psychosis and personality disorders. In my experience, confusion would be a prohibitive factor for the effectiveness of non-pharmacological interventions. Perhaps as I continue my research, I’ll find the answer to that question.
