NR 439 Role of Research and Searchable Clinical Question DQ

The practice of nursing is deeply rooted in nursing knowledge, and nursing knowledge is generated and disseminated through reading, using, and creating nursing research. Professional nurses rely on research findings to inform their practice decisions; they use critical thinking to apply research directly to specific patient care situations. The research process allows nurses to ask and answer questions systematically that will ensure that decisions are based on sound science and rigorous inquiry. Nursing research helps nurses in a variety of settings answer questions about patient care, education, and administration.

As you contemplate your role in the research process, read the following article.

I select the third case discussed in Kumar’s article “Type I diabetes Mellitus – Common Cases.” The 80-year-old retired army officer who lives alone that was on insulin and other OHAs, however, still experiencing hypoglycemic episode (Kumer, 2015).

Question:  Will long acting basal insulin, Degludec – Tresiba® , reduce the occurrence of hypoglycemic episodes in adults with uncontrolled Type II Diabetes Mellitus?

P: Adults with uncontrolled Type II Diabetes Mellitus

I: Long acting insulin Degludec

C: no use of long acting insulin

O: Reduce the occurrence of hypoglycemic episodes

T: no time element

The focus of this question is related to treatment of uncontrolled Type II Diabetes Mellitus in adults.

Studies have shown that Degludec has reduce the risk of severe hypoglycemia by 40% compared to other long acting insulins. Degludec has also reduced the risk of overnight severe hypoglycemia by 53% (Dmowska & Kennedy, 2017). Tresiba® is available in an easily use FlexTouch® pen. This would be beneficial for the retired solider in this case and other adult diabetic patient who have difficulty with medication administration/regimen
